Include food allergy information on the medical forms, or notify Meredith Renfro, merenfro@lindenhall.org, in writing at least two weeks prior to the start of camp so dining staff can be notified. This form is an editable PDF that must be downloaded and emailed back to merenfro@lindenhall.org.
